JOB SUMMARY

The Field Engagement Specialist is responsible for engaging STAR+PLUS Members and families in service coordination by facilitating communication, assisting in health systems navigating to promote access, encouraging the member to adopt healthy behaviors, and empowering the member, caregiver and /or family to interact with health/social services. Additionally, the Field Engagement Specialist supports the Member¿s assigned Case Manager through an integrated service coordination approach that involves engaging with the Member¿s community support. The Field Engagement Specialist actively outreaches and links members to behavioral health, primary care, and community-based services, especially to those who are historically hard to engage and/or the service coordinator is unable to reach through telephonic channels. The Field Engagement Specialist provides other services such as motivational interviewing, social support, informal counseling, advocating for individuals and community health needs, educating patients about the importance of routine health, wellness screenings, annual visits, monitoring and preventing the development of chronic health conditions. The Field Engagement Specialist will primarily work out in the field and at various events.

JOB SPECIFICATIONS 

  • The Field Engagement Specialist will conduct personalized support to help members achieve good health practices, conducts community outreach and home visits, collaborates closely with an interdisciplinary team, and consistently demonstrates a high standard of ethical behavior, professionalism, compassion, and confidentiality. They will ensure operational and department goals are achieved and strive to improve quality and business processes for assigned areas of responsibility.
  • The Field Engagement Specialist primarily works out in the field and at various events. Actively outreach and link members to behavioral health, primary care, and community services, especially those who are historically hard to engage or not linked to health and social services.
  • Maintains a professional behavior and exhibit excellent interpersonal skills.

QUALIFICATIONS:

  • Education/Specialized Training/Licensure: High School diploma or GED equivalent.
  • Certified as a Community Health Worker through Texas Department of State Health Services (DSHS) or other comparable organization required.  
  • Work Experience (Years and Area): Minimum of two 2-3 years of demonstrated experience in a community health setting: with patient navigation, care management, and/or any kind of direct service provision to patients in a community health/outreach, social resource agencies required.
  • Must be able to perform environmental assessments and social interventions in members private homes, which may have allergens or odors such as pet dander or tobacco smoke.
  • Experience in serving the Medicare population and/or older adults with disabilities population within diverse community health settings preferred.
  • Software Proficiencies: Must be technology literate in various systems (CRM/case management software such as Salesforce) and applications (i.e., Microsoft Word, Excel and Outlook). Moderate to advanced computer knowledge required.
  • Experience with and comfortable using keyboards, laptops, tablets, and other electronic and portable devices for documentation and telehealth visits preferred.
  • Other: Must have a drivers license and reliable transportation. Be comfortable and able to travel by personal automobile to multiple community locations each workday including private homes, hospitals, Clinics, Health Provider Offices and community sites.
  • Strong analytical, organizational, prioritization and time management skills.
  • Ability to work independently under minimal direction.
