JOB SUMMARY
This position is both telephonic and field-based. When you are not in the field, you will work remotely from home. The Peer Support Specialist is extremely vital to the interdisciplinary team as the Peer Support Specialist uses their own Recovery Lived Experience and training to assist families and consumers with defining Recovery goals and developing the skills and knowledge needed to assist our STAR+PLUS population. As a Peer Support Specialist, you will be collaborating with peers to help assess these needs, provide support, and facilitate referrals to social services as part of an interdisciplinary team. As the Peer Support Specialist, you will also provide the interdisciplinary team with information on community and Agency resources. The Peer Support Specialist must reside within a commutable distance to Harris County surrounding communities in Houston, TX.
JOB SPECIFICATIONS
- Identifies opportunities for engagement of members and their families in forming a supportive, recovery network.
- Collaborates with Interdisciplinary team (BH Case Management and BH UM) as a member advocate in discharge planning education, resolution of barriers, and service transitions.
- Acts as a resource for staff on decision making and problem solving.
- Be available to provide direct support to consumers being discharged from 24-hour care or to refer them to a support group.
MINIMUM QUALIFICATIONS:
- Education/Specialized Training/Licensure: 2 year degree with an Active Certification as a Peer Support Specialist required.
- Work Experience (Years and Area): 2 years of experience in health services or behavioral health field or in a peer support services role.
- Knowledge of the Recovery process
- STAR+PLUS experience preferred
- Software Proficiencies: Proficient with Microsoft Suites (Word, Excel, Outlook, Teams)
- Other: Ability to travel up to 50% of the time within Harris County area
