JOB SUMMARY

The Social Determinants of Health, Employment assists the Star Plus Team in implementing social determinants of health programs, including coordinating the life coaching efforts of their assigned clients and working with community partners to improve member well-being. The STAR PLUS Employment Habilitation is comfortable working with underserved populations and will be responsible for administrative duties of the Star Plus Team.

JOB SPECIFICATIONS AND CORE COMPETENCIES

  • Research developments in the fields of social needs in Houston and the surrounding areas.
  • Coordinates marketing material submission for star plus related programs and initiatives including but not limited to newsletters, flyers, videos, social media promotion, surveys, etc.
  • Assists with developing processes to deliver star plus programs supporting member well-being.
  • Assists with review and analysis of member data to evaluate programs, interventions, protocols, and metrics that support improved well-being.
  • Collaborates with community partners to assist with needs assessments and address social, education and health needs of members.
  • Tracks the social needs of members and contacts members to make referrals to community programs
  • Documents all coaching and development sessions in the appropriate databases following protocol and form completion, in a timely manner.
  • Demonstrates Harris Health and Community Health Choice values, including trust, integrity, mutual respect, diversity, responsiveness, and caring service.
  • Actively contributes to achievement of departmental goals, as identified in Departments annual business plan, including specific departmental process improvement plans.
  • Other duties as assigned.


QUALIFICATIONS:

  • Education/Specialized Training/Licensure: High School Degree or GED required. Community Health Worker Certificate or 4 years of equivalent work experience in lieu of certificate required.
  • Work Experience (Years and Area): 2 years of experiences as a community health worker, health care navigator, resource navigator, or in social needs referrals required.
  • Some exposure to office practices and procedures preferred.
  • Software Proficiencies: MS Office (Word, Excel, Outlook)

  • Must be able to work with projects requiring attention to detail.
  • Good organization, critical thinking, and critical listening skills.
  • Ability to communicate with a diverse group of people.
  • Ability to work independently and to follow instructions.
  • Ability to travel throughout Harris County and surrounding counties.
  • Ability to work hours outside of normal workday.
